Call activity not recording

I have a 10 user single license. When I run a call activity from the extension menu...for any extension I choose I only get this past May. Current setting is for CSV. I tried changing to MySQL and the change won't save...just reverts back to CSV.

There have been a coupld of software upgrades...so that may have changed the game since May...but I am at a loss of how to get the activity to begin being recorded again...or is it maybe out there and I can't get to it? Any suggestion on making the switch to MySQL to hold?
