I'm having a problem with some extensions that we have had in place for a long time on one of our tenants. The customer called and said they couldn't dial from one extension to another. It only effects a few of their 50 or so extensions. Here's what happens when they try to call extension 321.

When I look at the extension in Thirdlane/Extensions it looks ok. I tried saving one without making a change. When I do that the extension number goes away. In other words the list of 50 extensions includes all of their extensions including this one, but the extension number on this line is blank. (Am I making sense?)
The only way we have found to cure the problem for an extension is to delete and re-add the extension. When we do that the problems seems to move to another extension.
I assume there is something wrong with one of the dial plan config files, but I can't find it. I restored extensions.include from last week and did a dialplan reload, but it didn't change anything.
Has anyone seen this? Any ideas?

Somehow some of the extensions were missing from [local-extensions-WR]. Not sure how that happened. Copied one of them back from a backup, and all is well with that one.
