Error - Missing Headers

Hello All,
When I try to create a new sip exten I get the following error:
Error - Missing Headers
Any ideas on what I should look at?

Debian 5.x
2.6.26-2-amd64 #1 SMP x86_64
Asterisk 1.6.2.13
ThirdLane PBX Manager 6.1.1.5

Hi Nic,
that was a while ago but I believeit was due to a perl upgrade that I had done a couple week prior. Hadn't been n the system since and took me a while to realize.
